ITEM # 2: Tan Son Nhut SUBJECT: Information/Personnel Security Program DISCUSSION: 1. The following discrepicies more noted in the Personnel Security Program. a. There was no indication that the report of audit/inventory of the Top Secret Account within the 460th TRW had been reviewed by the commander or office chief, and there was no letter designating a Top Secret Control Officer Agent, however, Sgt Brown was performing this function. (Paragraogs 6-25c and 6-24d, AFR 205-1). b. TSN Regulation 136-1, "Emergency Destruction of Air Munitions," did not contain downgrading/declassification notations and was not annotated to show what portions were or were not classified. Confidential code word lists at CSC, did not contain predetermined downgrading/declassification instructions, "Attack Plan of U.S. Quick Reaction Forces on Base," classified Confidential was not annotated to show what portions were classified, and annexes to this plan did not contain downgrading/declassification instructions. Classified correspondence originated by BCE did not in all cases contain proper annotations to show what were or were not classified. (Paragraph 3-6 and Figure 3-1, AFR 205-1.) c. AF Form 502, "Persons Responsible For Safe," did not contain the date of the last combination change, office symbol, safe number and location, and was not attached to the front of the container. AF Form 308, Room or Area Security, Inspection Record," was not being used. (Paragraphs 5-3d)1) and 5-4(c), AFR 205-1) d. The record copies of classified messages orginated within the 377th Cmbt. Spt. Gp. (BSP, BDCM and USAF Dispensary) were not annotated to show the reason for classification and that information in message is classified and what is not. (Paragraph 3-51, AFR 205-1) e. Secret Document "Base Requirement - Resources Automated Project (BRRAP) CINCPAC, RCS: 4000-7, on hand in the BDCM Office was not controlled by AF Form 310: (Paragraph 7-6, AFM 10-1) f. The self inspection required by paragraph 1-6h, AFR 205-1 was conducted in the month of September as required, however; it did not show evidence of having been reviewed by Commander or Office Chief.
